Kirkwood Public Library U.S. Passport Application Acceptance Site
PLEASE TAKE NOTE:
On June 1, 2009, U.S. citizens must present a passport book, passport card, or other travel documents approved by the U.S. government to enter the United States from Canada, Mexico, the Caribbean and Bermuda at land borders and sea ports of entry.
The Information Services' staff is available to perform this service on a walk-in basis. Hours are: Monday 9:30am-8pm Tuesday 9:30am-8pm Wednesday 9:30am-8pm Thursday 9:30am-8pm Friday 9:30am-4pm Saturday 9:30am-4pm Sunday 1:30pm-4pm
In order to apply for a passport, please read pages 1 and 2 of "The Application for U.S. Passport or Registration" (Form DS-11). Copies of this form are available from the Reference staff at the Kirkwood Public library, at a designated post office or under Passports at http://travel.state.gov.
Some general guidelines for a successful submission of a passport application are:
- An unsigned completed application in dark black ink.
- Proof of U.S. citizenship - such as a state certified birth certificate, a certificate of naturalization, a certificate of citizenship or a previous valid U.S. passport. Photocopies of these documents will not be accepted.
- Proof of identity - a photo or physical description with a signature, such as a driver's license or previous passport.
- Two identical color photos that meet the U.S. Passport Agency specifications outlined on page 2 of the Application and have been taken within the last 6 months.
- For the renewal of a passport that was obtained for a child under age 16, both the passport and a state certified birth certificate are required.
- Fees for a passport:
Processing Fee $110 for an individual 16 years of age or older $80 for an individual 15 years of age or younger Check/Money Order payable to the U.S. Department of State. A seprate check or money order is needed for each application. No cash or credit card.
Execution Fee - $25 per application. Cash, Check, Visa or Mastercard.
Expedited Service Fee - $60.00 additional per application Check/Money Order payable to the U.S. Department of State. A seprate check or money order is needed for each application. No cash or credit card.
